Responsibilities
- Provide direct patient care in a home environment, including assessment, treatment, and education.

- Monitor patient progress and communicate changes to the care team.
- Maintain accurate and timely documentation of patient care activities.
- Educate patients and families on health management and disease prevention.
- Ensure compliance with all regulatory and company policies.
Requirements
- Current Registered Nurse (RN) license in the state of Michigan.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license OR reliable transportation for home visits.
