About this role

Maintaining Examination/Consult Rooms & Nursing Stations

1. Clean and stock exam rooms with both supplies and forms as needed. Inform supervisor/and or employee in charge of ordering supplies when supplies are low.

2. Review stored items for expiration dates and discard as appropriate. Dispose of expired supplies from exam tables and other areas were stored per appropriate policies/procedures.

3. Prepare instruments for sterilization per appropriate policies/procedures.

4. Set-up exam rooms for procedures as indicated but not limited to the following: thyroid biopsy, pap smears, thoracentesis, lumbar puncture, flex signs, bladder catheterization, casting and splinting, cerumen removal, etc.

5. Keep patient-care corridors neat and clear of fire hazards per protocols.

6. Remove and dispose of sharps containers as per current Joint Commission/CALS policy.

7. Keep nursing stations clean and clear of clutter, while maintaining HIPAA guidelines.

8. May take the lead to maintaining clinic-based equipment and inform supervisor/and or clinical engineering of problems with equipment.

9. May take the lead to inform supervisor on state of the facility as appropriate to patient care (i.e. light bulbs out).

10. Accurately record temperatures on all refrigerators and freezers.

Assist physicians/patients for office visits.

1. Accurately take and record vital signs in patient’s medical record utilizing EPIC/Care Connect system.

2. Medications (anesthetic/not anesthetic). Correctly identify patients by using at least two patient identifiers Administer medications and injections as ordered by physician following the proper Rights of Administration and record medications in patient’s medical record. Administer TB and skin testing when ordered.

3. Recognize symptomatology of common medical illnesses and take proper precautions.

4. Respond to and participate in physical and emotional needs of the patient and their families.

5. Act as liaison between patient family and all health care providers.

6. Respond to observations and entries of the patient’s condition or life situations.

7. Prepare and assist physicians with the patient requiring special procedures.

8. Perform 12 lead EKGs when requested.

9. Recognize abnormal lab values and report to the physician.

10. Participates in identifying, developing and/or implementing strategies to ensure continuity of effective care between or among contact with health care team, according to critical pathways

11. Completes current allergies, updates current medications, reason for visit, chief complaint, family history, mini mental status, PHQ9 for depression, height/weight, BMI, intake/output, temperature, pulse, respiration, blood pressure, pain and/or pulse oximetry, head circumference as directed

12. Completes required documentation on flow sheets per clinic protocol

13. Assists with sterile/and or non-sterile dressing changes and wound care and specimen collections such as but not limited to sputum, urine, wound, stool, finger sticks, etc.

14. Assists in the identification of environmental hazards and reports to appropriate personnel

15. As directed, range refills of prescriptions, new prescriptions and or changes to existing prescriptions after obtaining medical records, locating physician. Call the pharmacy with the approval and document all of the above information into the Medical Record.

16. Anticipate the needs of physicians based on patient information or physician practice style.

17. Document supplies/medications given to patients at the time of service on charge document and submit to billing within 24 hours.

18. May take the lead to coordinate safe and timely admission of patients to the hospital if and when necessary.

19. Perform wound care including removing sutures/staples and dressing as ordered by physician.

20. Perform procedures and tests such as but not limited to all POCT performed in the office, such as Accucheck, dipsticks, occult blood, blood cultures, INR, Rapid Strep, Hemocue, Mono test, pregnancy and perform procedures such as but not limited to , CPR, ear lavage, peak flow measurements, EKG, medication administration, bladder catherizations, oxygen administration , etc as per physician orders.

21. Print lab and diagnostic information, as needed by physicians.

22. Gather and organize patient charts daily and ensure correct charting procedures.

23. Maintain, promote and model a team approach to patient care

24. Assist with follow up for referrals to specialists.

25. Review chart prior to visit to determine if reports are pending, obtain.

26. Perform and assist high quality limited X-rays, where applicable, while adhering to all UCLA Radiology guidelines.

27. Maintain accurate records of Quality Control values as required for all waived tests performed.

28. Perform vision and hearing tests as requested.

29. Assist in educating patients in preparation for special examinations.

30. Perform venipuncture and perform waived tests dependent on laboratory equipment at facility as well as prepare specimens and requisitions for transport to the main laboratory.

Act as part of the team of nurses, MA’s, physicians and staff

1. Maintain teamwork and rapport with all areas within the Department i.e. reception/registration, appointment center, medical records, radiology, pharmacy, administration, secretaries, and physician and clinical colleagues.

2. May take the lead to orient new health care personnel to the physical set-up, equipment and general policies and procedures within the unit.

3. Assist physicians by reviewing requisitions and patient charts to ensure completeness for physician review.

4. Demonstrate professional attitude and appearance following the UCLA dress code.

5. Follow university guidelines in regard to telephone usage, equipment and supplies and report any known abuses to supervisor.

6. Communicate to supervisors any problems, issues and/or suggestions and any performance improvement opportunities for the practice.

7. Follow university and department guidelines in regard to attendance.

8. Provide back up to other administration support staff in other clinic areas during periods of vacation, extended illnesses or heavy workload.

9. Facilitates a calm, orderly and quiet environment.

10. Coordinates flows of communication between appropriate staff.

11. Respects customer/staff individuality and maintains compliance with HIPAA guidelines.

12. Consistently models exceptional interpersonal skills and caring behavior in working with patients, significant others and members of patient care team.

13. Effectively responds to the unique needs of pediatric, adolescent, adult, and/or geriatric patients as needs arise.

14. Assists with checking an stocking cart supplies and assists with charging of patient charge items.

15. Assists in tracking of the inventory and expiration dates of equipment and medications and sample medications and maintains the appropriate tracking of dispensing of sample medications.

16. May take the lead to participate in projects and communicates outcomes to staff and Lead MD/Clinic Manager.

17. May take the lead by assisting other members of the team while maintaining performance of individual duties.

18. Participates in work-related continuing education and communication activities.

19. Assists in involving the patient/family in design of their teaching plan.

20. Participates in providing informal teaching to staff and students in the clinic.

21. Utilizes structured patient teaching tools to meet the patient and family learning needs.

22. Participate in unit staff meetings and in-service sessions to contribute to process improvements.

23. Participate in the creating and updating of all clinical schedules & assigned duties list.

24. Maintain par levels for clinical and medication supplies and order weekly or as needed if sooner.

25. Follow proper chain of command and communications.

26. Respond to patient requests for educational information and provide.

27. TB skin tests, administer, measure results and read as positive or negative and document results appropriately in the medical record or in CareConnect.

28. Immunizations- Enter order, administer immunization, and document administration based on standing order/ protocol and document appropriately in the medical record, CAIR, or in CareConnect.

29. Place orders/requisitions for a test, referral or other non-medical related services based on defined protocol or per documented physician instructions for limited types of services in CareConnect, Tapestry, Access Express, etc or via additional on line processes.

30. Messages- Take and collect data from the patients.

31. Patient education- Reinforce and provide written educational and instructional information to patient/family under the supervision of the MD or provider.

Possess active license issued by the State of CA Board of Vocational Nurse Examiners with Intravenous Therapy and blood withdrawal certification.

Possession of active CPR BLS from the American Red Cross or American Heart Association.
