Licensed Vocational Nurse (LVN) - Behavioral Health
Make a meaningful impact in the lives of individuals experiencing homelessness and complex healthcare needs. We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team in a high-acuity, 24/7 care environment. In this role, you will provide direct patient care, coordinate services, support medication management, and work alongside an interdisciplinary team to help patients achieve their health and wellness goals.
Schedule
- Sunday - Thursday
- 3:00 PM - 11:30 PM
- Occasional overtime may be required based on patient care needs
What You'll Do
- Conduct patient intakes, room patients, obtain vital signs, and document medical histories.
- Perform patient screenings and assist with ongoing assessments and care coordination.
- Provide health education and support patients in understanding treatment plans and medications.
- Coordinate appointments and referrals with primary care providers, specialists, home health, dental, and therapy services.
- Manage medication reconciliation, verification, documentation, audits, and patient medication inquiries.
- Maintain accurate medical records and ensure compliance with regulatory and funding requirements.
- Support Clinical Support Specialists (CSS) and assist with shift coordination activities.
- Participate in quality assurance initiatives, including peer chart reviews.
- Assist with onboarding and training new team members.
- Deliver compassionate, patient-centered care to individuals experiencing homelessness, behavioral health conditions, chronic illnesses, and substance use disorders.
Required Qualifications
- Active California Licensed Vocational Nurse (LVN) license.
- Minimum of 2 years of LVN experience in behavioral health, psychiatric, or related healthcare settings.
- Experience with Electronic Medical Records (EMR) systems and clinical documentation.
- Strong patient assessment, intake, and care coordination skills.
- Knowledge of medical terminology and healthcare documentation standards.
- Experience supporting patients with mental health and substance use disorders.
- Excellent communication and patient education skills.
- Ability to thrive in a fast-paced, high-acuity healthcare environment.
Preferred Qualifications
- Experience with NexGen or similar EMR platforms.
- Familiarity with trauma-informed care and homelessness-focused healthcare programs.
- Previous experience in patient coordination, case management support, or community health settings.
- Strong organizational skills and the ability to manage multiple priorities.
- Interest in professional development and advancement opportunities.
Job Type & Location This is a Contract position based out of Sacramento, CA.
Pay and Benefits
The pay range for this position is $36.49 - $36.49/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
