Family Guide – FAM HUB – RPG-8

Role :

A FAM HUB Family Guide will provide support to individuals, children, youth and families in achieving their self-identified goals. The Family Guide will work closely with those accessing help through the FAM HUB to identify needs, provide resources, and facilitate access to various support services including housing, transportation, education, employment, food, clothing, mental health services, substance use treatment and parenting education.

Description of Duties and Responsibilities:

- Manage a caseload of families, helping them set and achieve self-identified goals.

- Facilitate strengths-based case planning with families and their children.

- Provide a menu of individualized potential solutions for those coming to the FAM HUB, while ultimately allowing the individual and/or family to direct what type of service they wish to engage in.

- Ensure all services are client directed, and recognize individuals and families are the experts in their own lives and goals.

- Connect families with community resources and services, ensuring they receive the necessary support and service coordination.

- Act as an advocate for children and their families, helping them navigate systems and access services effectively.

- Provide emotional support and guidance to families, fostering a trusting relationship.

- Teach parent education classes to biological, foster, and fictive kin parents using SafeCare and Parent Child Care curriculum.

- Utilize Motivational Interviewing to support positive behavior change.

- Act as a main resource to refer for treatment services including mental health and substance use services.

- Maintain accurate records of individual and family interactions and progress towards goals.

- Document all client data accurately, completely, and timely, in accordance with Crittenton Center and ACF project documentation guidelines.

- Work with project staff to achieve and maintain client caseloads. Upon discharge, provide follow up with families (monthly, semi-annually, and at 1 year post-discharge) to provide additional guidance and support as needed.

- Participate in project meetings, including case conferences, and supervision as scheduled.

- Complete training in evidence-based curriculum, as required.

- Participate in ACF and Crittenton Center training, community events, and other project activities as assigned.

- Ensure adherence to and the promotion of ACF priorities.

- Maintain client and employee confidentiality, abiding by all state and federal confidentiality expectations, including those established by HIPAA and Federal Substance Abuse Confidentiality Law (42 CFR Part 2).

- Contribute to the overall success of the grant by performing other duties and responsibilities as assigned.

Qualifications

Qualifications for Position :

Bachelor’s degree in social work, Psychology or a related field. Preferred experience with implementing evidence-based interventions.

Reports to: Project Director

Personal Qualities:

Excellent communication skills both verbal and written; excellent organizational and interpersonal skills; detail oriented.

Amount of Travel & Other Special Conditions:

Valid Iowa driver’s license, insurance coverage, and have an automobile. Typical hours Monday through Friday between 8:00 a.m. – 5:00 p.m.; may include some nights and weekends.
