About the Role:

As an RN in Navigation & Survivorship, you will help guide patients and families through the complexities of cancer care, serving as a consistent clinical resource from diagnosis through treatment and survivorship. You will work closely with physicians and the multidisciplinary oncology team to coordinate care, remove barriers, provide education, and connect patients with the resources they need.

This role is ideal for a nurse who values relationship-based care and enjoys combining clinical knowledge, patient advocacy, education, and care coordination to make a meaningful difference throughout a patient's cancer journey.

Schedule & Work Environment

- Full-time position, Monday-Friday, 8:00 AM – 5:00 PM, with one scheduled half-day each week.

- No weekends, holidays, or overnight shifts.
- Fast-paced outpatient oncology clinic supporting patients receiving cancer treatment and follow-up care.
- Collaborative team environment focused on exceptional patient care and clinical excellence.

Primary Responsibilities

- Serve as a clinical resource and point of contact for patients and families throughout diagnosis, treatment, follow-up, and survivorship.
- Assess patient needs and identify clinical, educational, psychosocial, financial, transportation, or other barriers that may affect care.
- Coordinate appointments, testing, referrals, treatments, and follow-up across the multidisciplinary care team.
- Provide education regarding diagnoses, treatment plans, symptom management, follow-up care, and survivorship.
- Communicate patient concerns and changes in condition to physicians and other appropriate members of the oncology team.
- Connect patients and families with appropriate internal and community resources.
- Support the development and coordination of survivorship care and follow-up.
- Advocate for patients and help facilitate timely movement through the healthcare system.
- Accurately document assessments, communication, education, interventions, and care coordination activities in the electronic medical record.
- Collaborate with physicians, nurses, social services, and other members of the oncology team to provide coordinated, patient-centered care.

Qualifications

- Current Illinois RN license required.
- Graduate of an accredited nursing program required.
- Current CPR/BLS certification required.
- Previous oncology nursing experience preferred.
- Previous patient navigation, care coordination, case management, or ambulatory nursing experience preferred.
- Strong clinical assessment, patient education, organization, and communication skills.
- Ability to develop trusting relationships with patients and families while helping them navigate complex care needs.

The compensation for this role includes a base pay range of $26.25 - $39.38 per hour , with actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through bonuses and other incentives. Base pay is only a portion of the total rewards package.
